Terms and Conditions

By booking an appointment with Harry Evans Sports Therapy, I agree to adhere to the following terms and conditions:

 

 I confirm that I am over 18 years of age. If I am under 18 years of age, a parent/carer must book on my behalf and also be present for the duration my appointment.

I agree to Harry Evans Sports Therapy's cancellation policy which states that cancellations any later than 48 hours prior to my booked appointment or failure to show up to my appointment renders me ineligible for any refund.

I agree to inform Harry Evans Sports Therapy if any circumstances arise that could lead to me being late for a booked appointment. I accept that failure to do so and arriving more than 15 minutes late will count as a missed appointment and I accept that will render me ineligible for any refund.

I agree that it is my responsibility to inform the therapist of any information relevant to my injury including past medical history and if there are any changes to this, it is my responsibility to inform the therapist.

By booking an appointment I agree to respect the therapist at all times throughout any contact and face-to-face appointments. If the therapist feels the need at any point, they have the right to terminate any appointment and refuse future bookings.

Please note, the therapist may refuse or terminate your appointment if these terms and conditions are not met. If such circumstances arise, there is no guarantee of refund.

I agree for my email to be used for communications from Harry Evans Sports Therapy, both regarding my appointment as well as newsletters, offers and any other email marketing.
